add a default service for PAMA requests

Before the connectathon, we would like to have a default service entry available for the order-select hook on the PAMA tab.

• enable a CI/CD pipeline to deploy the sandbox-cds-services.
• populate the default service to reference the public service.

sandbox-cds-services will build and deploy to GCP - https://github.com/cds-hooks/sandbox-cds-services/blob/master/.travis.yml . Are you looking for something else for that first bullet?

With the merge of PR 15 - does the sandbox need to be rebuilt or reloaded to, by default, connect to the new pama-imaging service?

My last (and only) time updating sandbox-cds-services, it deployed to App Engine, but I had to log into GCP to manually increase the percentage of traffic to shift from the old version to the new.
