Default to R4 FHIR Server #138
Description
The sandbox currently defaults to a DSTU2 FHIR Server. With newer FHIR versions released and US regulations pushing adoption for R4, it makes less sense to keep the sandbox defaulting to an older version of the FHIR spec.
