Document where issues should be opened

Given the number of organizations and repositories we have it is difficult for users to know where to open issues when they encounter problems.

We can use lapps.github.io as a catch-all and move issues to the proper location, but this should be documented on lappsgrid.org and on wiki.lappsgrid.org
