Manual Entry for Secret Key

Instead of just scanning a qr code, there should be an option to manually enter the secret key and store it just the same. A bit of ui changes may be needed, possibly a dropdown for the + button (on the top-left)

Requirements:

• Change UI for adding an application to MOS Authenticator
• Add an input box for manual code entry (replacement for qr code scan)
