Issues with comment duplication and formatting when tagging copilot for follow-up

Steps to Reproduce:

1. Add a comment for the coding agent, like @Copilot help add some comments to this file
2. ❓ Comment is duplicated

3. ❓ If I close the file and reopen, I only see one comment, and @Copilot isn't clickable:

4. If I manually open the PR webview from the GHPR view, the comment looks more as expected:

After Copilot completed its work, I open the updated changes from the chat editor again:

And the comment now looks as expected:

I've been able to occasionally repro. Possibly a candidate if I can figure out what's going on.

I found it, and it's been around for 4 years, so not a candidate. I'm not sure why it started now.
