Add remote mcp example for pingfederate

Implements a Model Context Protocol (MCP) server with PingFederate OAuth authentication, deployable to Cloudflare Workers.

Does not use cloudflare-oauth-provider. Uses DCR for client onboarding and token-exchange for calling downstream API.
