Besides other changes, this commit ensures the generated HTML doc for HexDocs.pm will become the main source doc for this Elixir library which leverage on ExDoc features. List of changes: * Use common source url * Update gitignore * Set readme as main html page * Fix markdowns * Add changelog to html page * Add changelog link to hex info page * Badges and more badges! * Use and set latest ex_doc
