KVM_FEATURE_MSI_EXT_DEST_ID allows the guest to encode bit 8-14 of
x2APIC ID into bit 5-11 of MSI address. Theoretically, up to 32768 CPUs
are supported with such hack.

When booted in x2APC mode, Linux is able to bring up CPUs with x2APIC ID
> 254.
