docs/schema/: create individual pages for all codelists

• Move codelists to individual pages
• Add a badge below each codelist page title to indicate whether the codelist is open or closed. The badges can link to the sections of the codelist landing page that explain open and closed codelists.
• Order the codelist pages alphabetically in the navigation
• Propose a grouping and ordering for the links from the codelist landing page to the individual codelist pages

Will wait for #1660 and #1680 to be merged prior to fixing links broken by this PR and then converting it from draft

Have used the 'primary' badge for closed codelists (blue with white text) and the 'secondary' badge for open codelists (grey with white text). Other colour options are available.

Used danger badge (red with white text) for deprecated badge
